We welcome all new patients to our practice.

We do accept patients who live outside our catchment area. Please see our 'out of area' page. Please note this may not be suitable for all people.

Online Registration

We encourage anyone who can to register online  - it is quicker and can be done at your convenience 24 hours a day. Please allow three working days for us to process your registration application (after submitting your online registration - with the required documentation) then book an appointment if you require medication. If you need anything sooner please call the surgery and speak to a member of reception. 

N.B Please be aware that if you are on highly addictive or controlled medications (i.e. tramadol, diazepam, temazepam etc) that we are unable to prescribe these medications until we have your old medical records, so please liaise with the reception to ensure we have these before booking an appointment. Please also note that we do not prescibe out of accordance with national and local medicines management guidelines.

Please note that if you are outside of the practice map area and are likely to require home visits i.e. housebound/mobility issues, then please register with a practice more local to you.

To provide more holistic care including safeguarding the family, families should be registered at the same practice. When registering chidlren with us, at least one parent should be registered with us.

 

In-Person registrations

Alternatively, registration can be done in person at reception during our normal opening hours

In order to facilitate the in-person registration process, please complete the pre-registation section and contact us to send you a health questionnaire.

We are required to have sight of proof of ID that includes photo ID. You will need to bring:

  • One proof of residency (e.g. rent book or utility bill. Bills must be dated within the last three months. Mobile phone bills can not be accepted)
  • Current medical card or substitute GMS1 form, available from reception.
  • National health service number if available
  • Completed practice questionnaire, available from reception
  • Photo identity such as passport or drivers licence.
 

Your Named GP

The surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.

We need to divide our patients evenly between all of our doctors, so they can manage their workload. To do do this fairly and impartially, we have selected patients by surname to match to a doctor.

We know that some patients will wish their named GP to be a particular doctor. Where a patient expresses a preference as to which doctor they have been assigned, the practice will make reasonable efforts to accommodate this request, but we would like to reassure patients that this system does not prevent you from seeing any GP in the practice as you currently do. Your named GP will not be available at all times.

You do not need to take any further action and will be informed of your named GP in due course. In the meantime, if you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
